O’Keefe, who sits fourth on the all-time games record list for the Swans with 286, will feature in a lap of honour at half-time of the Swans final home and away match of the season.
While O’Keefe has not played senior football since round four, he is yet to make a decision about his future in the game.
The 33-year old said he understood he was unlikely to play senior football for the Swans again, but wanted to farewell and thank the Swans faithful.
“I have had 15 fantastic years at the Swans and it is important to me that I am able to thank all the members and fans who have supported me,” he said.
“It has been a challenging year, but I have enjoyed working with the young players in the reserves and look forward to helping that group challenge for the NEAFL premiership in the coming weeks.
“I will have a look at my options at the end of the season, but it is important to me that I get to thank the Club, all my teammates and importantly, all of the supporters.”
Sydney Swans coach John Longmire said O’Keefe was a champion of the football club and deserved a great send-off.
“Ryan’s record speaks for itself. He has played 286 games, won two premierships, a Norm Smith medal and a Club Champion award,” Longmire said.
“He is one of the toughest competitors I have come across in the game. Very few players share his professionalism, his drive and will to win.
“As it is with many of the very best players, most of Ryan’s best games came in finals.
“His performances throughout the 2012 finals series, where he averaged 11 tackles a game, were remarkable, and he capped it off with an outstanding Grand Final performance and a Norm Smith medal.
"Despite the challenges this season, he has handled the situation impressively and been a great mentor to our young players.
“He is a champion of the Swans and I hope all of our members and fans can get along to Saturday afternoon’s match and thank Ryan for his enormous contribution to our club.”
O'Keefe was drafted to the Swans as a fourth round selection (pick 56 overall) in the 1999 National Draft.
He made his senior debut in round 10, 2000 against the Western Bulldogs.
He was All-Australian in 2006 and Club Champion in 2009.
